We enjoy taking pictures/video to help promote our programs and to show others what happens in our Mosaic Kids ministry here at Citylight Mosaic Church. To use pictures/video of your child(ren) for publicity or promotional purposes, we need and would like, your permission. The statement below grants us that permission.

I/We give permission for the staff and/or volunteers at Citylight Mosaic Church to take pictures and/or audio-visual recordings of my child(ren) to be used in future promotion of Citylight Mosaic Church and her activities.

Your child will be cared for as if he/she were our child. Every precaution is taken for the safety and health of your child, but in the event of sickness, Citylight Mosaic Church, its staff, and its volunteers are hereby released of any liability.
